Erectile dysfunction (ED) is a common condition characterized by the inability to achieve or maintain an erection during sexual activity. As men age, ED becomes increasingly more prevalent. As much as 40% of men in their 40s already have ED and it’s not an easy conversation to have, even with their doctors. It affects their sex life, their sexual performance, and their confidence level. In many cases of ED, there is a problem with blood flow; either a lack of blood flow in, or too much blood escapes. Either can result in poor erection quality.

Extracorporeal shock wave therapy (ESWT), commercially known as GAINSWave®, is an off-label treatment that uses an FDA Approved device which may improve some of the problems which can cause ED. Research shows that ESWT may break up microvascular plaques and may even aid in the generation of new blood vessels. Many men report significant benefits with ESWT treatments. Even if they don’t have ED, many men have used the treatment for performance improvement. Hypogonadal men, treated with testosterone, report significant improvements from the combination of testosterone therapy and ESWT.

FREQUENTLY ASKED QUESTIONS

I feel too young to have erectile dysfunction, can you help?

Yes. Erectile dysfucntion (ED) can be an embarassing problem to bring up. It’s ok if it takes you until the end of the appointment to even ask, if your provider doesn’t bring it up first. There are a number of prescribed medications that are effective for treated ED in men. But if a man developds ED, he should also be screened for cardiovascular disease becuase ED is a red flag that there is possibly a blood flow problem.

Does shock wave therapy actually work?

The research shows that it is effective in about 75% of individuals. From clinical practice, it’s been observed that the healthier an individual, and the better they respond to oral ED medications (e.g., Viagra, Cialis), the better their response to ESWT. 

Does the device actually shock?

No. Think of the shock wave like a focued, high-energy sound wave. The device used is a called extracorporeal (meaning outside the body) shock wave therapy (EWST). In ESWT, a soundwave can be created a number of different ways. In a radial device, such that is used in the GAINSWave protocol, a diffuse wave in generated by the handpiece from bullet traveling through a chamber an impacting the strike-face of the treatment tip. In a focus wave device, a cylindrical wave is generated by a small underwater explosion inside the handpiece. This wave is focused through a reflector and transmitted into the tissue of the penis at a therapeutic depth.

Who is not candidate?

Shock wave therapy is best used for vasculogenic ED, or ED that comes from a blood flow problem. The men who are not candidates are those with neurogenic ED, such as those who have suffered nerve damage from radical prostatectomy, or men who have an implantable penile device.

Treatments will be delayed if someone has symptoms of an active sexually transmitted infection or if they have open sores or lesions in the genital area.

Other considerations that your healthcare provider will take are if you have a coagulation disorder or are on anticoagulants, if you have Peyronie’s disease, a history of cancer, or a history of blood clots.

Does it hurt?

There can be some discomfort from the treatments as each man’s degree of sensitivity varies and each has a different tolerance for pain. It is not uncommon to prescribe a numbing cream (benzocaine, lidocaine, tetracaine) to ensure that someone stays comfortable for the duration of treatment. Numbing cream is actually recommended for treatment with the radial wave device used in the GAINSWave treatment. 

Is it safe?

Yes it is safe. The treatments are completed with a medical device which means that the person performing your treatments should be a licensed healthcare provider or an individual who has been certified in the device’s use and is under the supervision of a licensed healthcare provider. 

Are there side effects?

Typically there are none and the procedure is well tolerated. There is a possible but low risk of pain or discomfort during the treatment, pain or swelling for a brief period following treatment, temporary localized numbness, tingling, or decreased sensation in the treatment area, reddening of the skin, soreness of the genitals from their manipulation by the provider performing the treatments. There is a consent form that is signed prior to the procedure with all the details but most men are back to business as usual that afternoon.

I was prescribed testosterone, will that also help?

In our experience, yes it will. Low testosterone levels can cause erectile dysfunction and when you replace the hormone, you can regain function. Our patients report that testosterone paired with GAINSWave has had the best effect.

I heard about those pills, you know, the blue ones.

It is common to be prescribed medication as part of the treatment plan to address erectile dysfunction. One of the goals during the treatment protocol is to achieve an erection for at least ten minutes, twice daily, for the time period that someone is having treatments. This can best be achieved with a prescribed medication such as tadalafil (Cialis) given that is has a long half-life and many men report having more spontaneous erections with this medicaiton.

Labs are not typically performed with a GAINSWave consult but this is an option. We  typically like to look at your hormone levels, inflammatory biomarkers, cardiovascular and diabetes risk markers, and genetics.

Step 3

 CONSULTATION

You will be booked for your  initial medical consult with one of our board-certified  providers to review your labs, talk about your medical history, and to discuss a treatment plan tailored to you and your personal goals. 

Step 4

PLAN

Based off of your concerns, goals, a review of your labs, and what your provider sees as medically significant, a customized plan is put together. Medications, if needed, are prescribed that day to your pharmacy.
